Quick Facts:

• Monday through Friday until shift is complete
• Productivity expectation of at least 30 units per week
• Home health and adult care patients
• 1 year homecare experience with Homebase system required
• 1 year home health care and OASIS experience required
• 1-2 years acute care or rehabilitation setting experience required
• Current driver's license required for home visits
• 13-week assignment length
