Physical Therapist - Home Health

Location: Cape Girardeau, MO (Hybrid- includes travel within Cape Girardeau County)

- Conduct initial and ongoing comprehensive assessments of patients in their homes

- Develop individualized treatment plans in collaboration with physicians

- Administer physical therapy treatments to relieve pain, improve function, and maximize mobility

- Educate, supervise, and counsel patients and families on therapeutic care plans

- Monitor and document patient progress and communicate updates to physicians

- Complete and submit accurate evaluation data, progress notes, and discharge summaries

- Participate in discharge planning with the interdisciplinary team

- Travel within the designated service area (Scott, New Madrid, and Stoddard Counties)

- Graduate of an accredited Physical Therapy program (recognized by the A.M.A. and APTA)

- Licensed or registered to practice Physical Therapy in the state of Missouri

- Minimum of 1 year of professional experience preferred

- Professional liability insurance required

- Valid driver's license, auto insurance, and reliable transportation

- Residency in or near Scott, New Madrid, or Stoddard County preferred
