JOB REQUIREMENTS: POSITION SUMMARY: This position provides a combination
of speech therapy services, training and education, and case management
to eligible infants and toddlers and their caregivers; providing
therapeutic and educational intervention to promote development and
functional participation within a natural environment. ESSENTIAL
FUNCTIONS: The following duties are normal for this position. These are
not to be construed as exclusive or all-inclusive. To perform this job
successfully, an individual must be able to perform each duty
satisfactorily. Other duties may be required and assigned. -Provides
speech therapy services to program participants in the clients\' homes,
daycare, and other natural environments. Provides coaching to families
and caregivers to address child\'s delays. -Completes documentation
related to services provided to families, including service plans, and
prior authorizations for insurance. -Submits referrals to providers or
community partners, resources or agencies for additional services.
-Facilitates service plan meetings with providers; maintains contact
with providers to monitor plan and schedule evaluations and other
meetings; provides support to family in service plan implementation.
-Works with staff, parents and caregivers in screening, teaching and
monitoring the developmental skills in children who are age eligible.
-Researches and provides resources to families related to their child\'s
diagnosis or family needs. -Performs standardized evaluations;
participates in multi-disciplinary team assessments; interprets results;
prepares reports and service plans. -Provides information to families
and caregivers assisting them in understanding child developmental
needs. -Conducts intake visits, including developmental screenings.

QUALIFICATIONS: MINIMUM QUALIFICATIONS: EDUCATION AND/OR EXPERIENCE
REQUIREMENTS -Masters degree in Communications Disorders or related
program. -Speech Therapist License preferred; Clinical Fellowship Year
(CFY) candidates will be considered. -State of Wisconsin Speech
Therapist License required within 1 year from date of hire. -Certificate
of Clinical Competence by ASHA required within 1 year from date of hire.
-Two years experience as a Speech Therapist with children preferred, but
not required. -Appointment will be conditional upon successful
completion of a caregiver and criminal background check. \*\*\*\*\*
