07/07/20251299 E Commercial BlvdSte 202, Oakland Park, Florida, 33334-4806, United States of America This role will join the IKC Flex Team. This team does not have dedicated patient loads but instead provides much needed support for staffing and LOAs in assigned geographies.

Position Details:

• Location: Remote. Work from home (telehealth)
• Monday- Friday schedule with the ability to accommodate patient and nephrology partners availability. Must be flexible.
• Full-time position.
• Quickly build empathetic relationships with patients and families.
• Coordinate care for patients care including care transitions, management of complex/at risk patients, managing ongoing needs and establishing a treatment plan in partnership with the care team.
• Identify and address social determinants of health including medical, emotional, education, community, and financial concerns.
• Coordinate for streamlined/effective transition of care when needed.
• Assesses patients and work to identify unreported medical conditions or changes that may lead to adverse outcomes.

Requirements:

• Current Compact RN License is Required
• Minimum of 1 year of experience as an IKC Case Manager Required
• Ability and willingness to work across multiple time zones

Salary/ Wage Range

$75,200.00 - $110,400.00 / yearIf a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.

 New York Exempt:
New York City and Long Island: $64,350.00/year
Nassau, Suffolk, and Westchester counties: $64,350.00/year
Remainder of New York state: $60,405.80/year

New York Non-exempt:
New York City and Long Island: $16.50/hour
Nassau, Suffolk, and Westchester counties: $16.50/hour
Remainder of New York state: $15.50/hour


Washington Exempt: $77,968.80/year

Washington Non-exempt:
Bellingham: $17.66/hour
Burien: $21.16/hour
Unincorporated King County: $20.29/hour
Renton: $20.90/hour
Seattle: $20.76/hour
Tukwila: $21.10/hour
Remainder of Washington state: $16.66/hour
