Description
  • Thoroughly assess patients in either an inpatient or outpatient context and tailor individualized treatment strategies
  • Apply physical therapy modalities focused on enhancing mobility, building strength, improving balance, and restoring function
  • Partner with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation professionals to achieve exceptional patient outcomes
  • Provide education to patients and their families about treatment goals, discharge planning, and at-home exercise routines
  • Maintain clear documentation of evaluations, treatments, and progress, adhering to facility protocols and regulatory standards
  • Engage in collaborative care rounds and interdisciplinary discussions to ensure seamless transitions of care
Education
  • Doctorate in Physical Therapy (DPT) or its equivalent from an accredited institution
Certifications
  • State licensure required for practice as a Rehabilitation Specialist
  • CPR certification preferred
