Position Title: RN Case Manager
Department: Medical Case Management
Reports To: Director or Program Operations Supervisor
POSITION SUMMARY
The HIV RN Case Manager will provide primary HIV nursing care and will provide the medical care coordination services for the adult HIV population.
This position requires the full understanding and active participation in fulfilling the Mission of Tarzana Treatment Centers, Inc. (TTC). It is expected that the employee demonstrate behavior consistent with the Core Values. The employee shall support Tarzana Treatment Center's strategic plan and the goals and direction of the Performance Improvement Plan (PIP).
CATAGORIES OF DUTIES
- Nursing assessment, evaluation and follow-up
- Patient acuity assessment
- Triage as appropriate
- Consultation and ongoing communication with primary practitioner
- Comprehensive treatment plan
- Patient counseling in HIV/STD prevention, HIV education and treatment adherence
- Provision of any service which requires substantial specialized nursing skill
- Initiation of appropriate preventive nursing procedures
- Screening for substance abuse and mental health issues
- Coordination of other services to assist in the medical management of patient in conjunction with medical coordination such as specialty referral coordination and follow-up, case conferences, patient retention services, cooperation with social workers, and other case mangers and participation in Quality Management.
EDUCATION/TRAINING/EXPERIENCE
Graduate of an accredited RN School of Nursing. Two (2) year recent acute care experience preferred. Excellent written and verbal communication skills
LICENSES/CERTIFICATIONS
Current California RN License Current BLS Card
