Job Objective: The Occupational Therapist is responsible for assisting in the care and rehabilitation of designated home care patients. The therapist plans, administers, and evaluates individual treatments of assigned patients to maximize the patients independence and safety in activities of daily living and other functional activities.

Duties and Responsibilities:
A. Patient Care
1. Demonstrates understanding of the theory and practices of accepted Occupational therapy treatment.
2. Acts as the patients advocate and protects patients confidentiality.
3. Based on the evaluation, develop treatment programs with appropriate treatment objectives which are mutually agreed upon by the patient and physician.
4. Administers standard diagnostic or prognostic assessments of occupational performance components, including neuromuscular, sensory-integrative, cognitive, and psychosocial skills.
5. Administers assessment and treatment of occupational performance, including independent daily living skills and functional abilities, work adjustment, play/leisure abilities, and educational and social skills.
6. Periodically evaluates the patients response to treatment and alters the program as appropriate with the physicians approval.
7. Communicates with Intake area and/or physician for Durable Medical Equipment needs, patient problems or other issues, as appropriate.
8. Demonstrates knowledge regarding special needs and behaviors of age specific groups and demonstrates proficiency in required age specific competencies.
9. Maintains own schedule for patient visits and communicates visit plans with Director of Patient Care Services and/or Case Manager.
10. Utilize disease state management programs once established.
B. Communication
1. Able to communicate effectively while maintaining good working relationships with co-workers, managers, and office staff.
2. Acts as a resource for patients to ensure understanding and compliance of the assessment plan.
3. Participate in interdisciplinary case conferences, in-service programs, and committee meetings.
4. Communicates to Case Manager, physicians, and/or Director of Patient Care Services changes in the patient condition and needs.
C. Documentation
1. Prepares complete, concise and clear documentation of planned interventions, goals, patients response to treatment, changes in patient condition and coordination with other services.
2. All documentation is completed timely within agency policies.
3. Effective oral and written communication skills for reporting and recording clinical findings.

D. Education
1. Participates in age-specific educational and training programs.
E. Case Management
1. Responsible for the overall planning, ongoing care, and discharge planning of the patients care.
2. Responsible that the patients plan of care meets all regulations, and that payment is authorized when necessary.
3. Responsible for notifying physicians of changes to the plan of care and/or barriers to plan of care.
4. Responsible for communication with other disciplines as appropriate.
5. Responsible for maintaining an accurate schedule per agency process, including appropriate clinical needs, completing or assigning recertification within CMS requirements, and scheduling supervision of Home Health Aide (CHHA).
6. Responsible for reporting patients care, progress and barriers to plan of care in Case Conferences and as needed to DPCS.
F. Supervision
a) Responsible for Supervision of CHHA by establishing CHHA plan of care, performing and documenting CHHA supervisory visits every 14 days, and updating the CHHA plan of care as needed.

Non-Essential Duties:
Participates in Quality Improvement (QAPI) process as requested in Utilization Review (UR), chart audits, and PI committees.
Participates in orientation of other Occupational Therapist personnel.
Other duties as assigned.

Ability to take ownership of patients plan of care including establishing an appropriate plan of care, overseeing that care is done according to plan of care, communicating with physician and revising plan of care as needed.
Ability to understand basic Home Health regulations and the financial impact of care being provided.
Ability to oversee that all aspects of the patients care meet all regulations, and that payment is authorized as required.

Education and Experience:
Education
Completion of an AOTA accredited Occupational Therapy educational program

Experience
At least 1-year recent relevant clinical experience as an Occupational Therapist
Home Health experience preferred.
Must have strong clinical skills in order to function independently in the field.

Required Licensure and/or Certifications:
Current CPR certification.
Maintains a valid California drivers license and evidence of current auto insurance.
An Occupational Therapist who began practicing as an OT on or after January 1, 2010, must be:
1) Licensed as an occupational therapist in the state in which practicing.
2) Graduated from an occupational therapist education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or a successor organization.
3) Eligible to take or have passed the entry-level certification examination for occupational therapists developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT)
An Occupational Therapist who began practicing as an OT after January 1, 2008, but on or before December 31,2009 must be:
1) Licensed as an Occupational Therapist in the state in which practicing.
2) Graduated from an occupational therapist education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) or a successor organization.
3) Eligible to take or have passed the entry-level certification examination for occupational therapists developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT).
An Occupational Therapist who began practicing as an OT on or after January 1, 1978, but before January 2, 2008, must:
1) Graduated from an occupational therapist education program accredited jointly by the Committee on Allied Health Education and American Medical Association and the American Occupational Therapy Association or
2) Eligible for the National Registration Examination of the American Occupational Therapy Association or the National Board for Certification in Occupational Therapy.
An Occupational Therapist who began practicing on or before December 31, 1977, must have:
1) Two years of appropriate experience as an occupational therapist.
2) Achieved a satisfactory grade on an occupational therapist proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service.
An Occupational Therapist who was educated outside of the United States must have:
1) Graduated from an occupational therapist education program accredited as substantially equivalent to occupational therapist entry level education in the United States by an allowed credentialing body.
2) Completed the entry-level certification examination for occupational therapists developed and administered by the NBCOT, and if on or before December 31, 2009, must be licensed as an Occupational Therapist in the state in which practicing.
