Speech Language Pathologist - Homecare

JOB SUMMARY The Speech Language Pathologist is responsible for providing speech/language/dysphagia therapy to Home Care/Hospice patients and primary case management when indicated. Responsible for timely and accurate communication and documentation relative to speech therapy delivery. Promotes and develops speech/language services in Home Care/Hospice. MINIMUM QUALIFICATIONS Education: See Licensure/Certification/Registration requirement. Experience: Two (2) years previous experience. Licensure/Certification/Registration: Certificate of Clinical Competency (CCC) in Speech/Language Pathology from ASHA (American Speech and Hearing Association). Minnesota license in Speech/Language Pathology.# Wisconsin license in Speech/Language Pathology within six (6) months of hire.# Wisconsin Background Study upon hire and renewed per Wisconsin state law.
