Description- Assess patients in either an inpatient or outpatient context, developing tailored treatment plans
- Deliver physical therapy interventions centered on mobility, strength, balance, and functionality
- Collaborate with nursing staff, physicians, case managers, and other rehabilitation professionals to enhance outcomes
- Instruct patients and their families regarding therapeutic objectives, discharge planning, and home exercises
- Maintain documentation of evaluations, treatments, and patient progress following facility and regulatory standards
- Engage in care team rounds and interdisciplinary meetings to ensure cohesive care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program
- Must have state licensure as a Physical Therapist
- CPR certification is preferred
- Specialized certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory
