Responsibilities of Physical Therapist (PT):

• Perform physical therapy evaluations and develop individualized treatment plans

• Provide skilled therapy services to support residents mobility, strength, and functional independence

• Collaborate with nursing staff, rehab team, and physicians to ensure coordinated care

• Educate residents and caregivers as needed

• Maintain thorough and timely clinical documentation in compliance with all regulations

Qualifications for Physical Therapist (PT)

• Current Physical Therapist (PT) license in the state of Ohio.

• Previous SNF experience preferred, but not required
