Colorado Visiting Nurse Association - Physical Therapist - Home Health (Sign-on bonus $3,500)

We are hiring Physical Therapists to serve patients in the Southeast Denver metro area, including Aurora, Centennial, Parker, and Lone Tree. Position Details:
Schedule: Monday–Friday, 8am–5pm
Status: Full-Time, 25 visits a week
Setting : Home Health
Territory : Southeast Denver metro (visits clustered for efficiency)
On-Call: Rotating weekday/weekend schedule
Per Visit Rates: Start of Care: $150; Evaluation: $110; Recertification w/o Oasis: $91; Recertification w/ Oasis $114; Resumption of Care: $110, average full-time earnings between $80,000–$100,000/year
Sign-On Bonus : $3,500 ( 3 payments within 1 year allocation )
Mileage Reimbursement : Sixty-two and a half cents per mile – one of the highest in the industry!

Rewarding Work You Will Do
- Provide physical therapy evaluation, treatment, and reassessment for home health patients under physician orders.
- Conduct OASIS assessments and document using our streamlined EMR system.
- Educate and train patients, caregivers, and staff on mobility, equipment, and home safety modifications.
- Supervise Physical Therapy Assistants and collaborate with CNAs, nurses, and case managers.
- Participate in discharge planning and care coordination.

What We Are Going To Love About You
- Minimum Education: Graduation from a four-year college or university, including completion of a physical therapy education program.
- Minimum Experience: At least one year of physical therapy experience required, with two years of SNF, hospital or inpatient rehab preferable. Home health experience also preferred. Other requirements may vary by case or department needs.
- Required License: Must have active Colorado PT license. Current CPR certification based on individual affiliate requirements. Valid Colorado driver’s license and proof of automobile insurance in accordance with state statute.
PHYSICAL REQUIREMENTS:
- Ability to lift/push/pull minimum of 35 lbs

Care Synergy provides mission support services to better equip Colorado Visiting Nurse Association to operate as a distinct and independent organization while working together to share best practices and serve more Coloradans along the Front Range.
