Description
  • Conduct thorough evaluations of patients in both inpatient and outpatient environments, tailoring customized treatment plans to their needs.
  • Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality.
  • Engage and collaborate with a multidisciplinary team of caregivers, including nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes.
  • Guide patients and their families through therapeutic goals, effective discharge planning, and appropriate home exercise routines.
  • Maintain detailed documentation of evaluations, treatments administered, and patient progress per regulatory and organizational standards.
  • Actively participate in care team rounds and interdisciplinary meetings to ensure seamless care coordination.
Education
  • Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution.
Certifications
  • State licensure as a Physical Therapist .
  • CPR certification is preferred.
  • Specialty certifications (e.g., neurological, geriatric) are a valuable addition but are not mandatory.
Skills
  • Relevant experience is strongly preferred.
