Travel Allied - Physical Therapist (PT)

Epic Travel Staffing is hiring a Travel Physical Therapist Home Health

Shift: Days / 8a-5:30p, 85, 40hrs per week, M-F

• WI License
• 1 year experience
• Minimum Experience: Home Health experience, training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis
• Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy
• Masters Degree in Physical Therapy required
• AHA BLS

• Float required
• Must obtain NPI number if candidate does not have it already
• Charting/Equipment: Laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication
• Unit Specific Skillset/Unit Overview: Evaluates, treats and plans patient care in accordance with Physical Therapy Standards of practice. Population served includes adult and geriatric patients. Documents to meet regulatory and reimbursement requirements. Meets productivity expectations of an average of 4.5 visits per day
• Scrub Colors: any or Business casual
