We provide Rehabilitation Services including Physical Therapy, Occupational Therapy, and Speech Therapy, at the main hospital as well as multiple outside outpatient clinics. This full-time outpatient position offers flexible days and hours, time off and reimbursement for CEUs, licensure renewal, and mentorship for new graduates. We pride ourselves on treating patients 1:1 with no patient overlap for the full 1-hour session. In addition to having an open-door policy, we encourage a good work/life balance.a

Position Purpose:
Evaluates and treats patients using the principles and practices of physical therapy in accordance with the patient's age, developmental level and diagnosis.
Essential Functions:
Evaluates patients, establishes goals and develops individualized treatment plans.a
Provides direct patient treatment based on current physical therapy practice and revises treatment plan based on the patient's response.
Provides education to patient and family regarding patient abilities and need for assistance and continued therapeutic intervention.
Consults with physicians on patient's evaluations, progress, and complications with treatment.
Maintains medical and departmental records according to department policies and procedures.
Participates in patient care conferences, department meetings and department and hospital in-services.
Communicates with other professional staff and patient's family/support system to provide continuity of care.a
General Job requirements:
Doctor of Physical Therapy, Bachelor’s degree prior to 2015 accepted
Illinois PT license
CPR Certification
Experience in an outpatient settingapreferred, but not required
Bilingual English/Spanish a plus, but not required
Ability to communicate with staff at all organization levels
Effective written and verbal communication skills
Customer service skills
