Home Health Physical Therapy Assistant

Meritus Health is seeking a compassionate and skilled Physical Therapy Assistant (PTA) to join our Home Health team. In this role, you'll work under the supervision of licensed physical therapists to deliver high-quality care and help patients regain optimal physical function in the comfort of their homes. You'll collaborate closely with rehab team members, physicians, and caregivers to ensure continuity of care and successful patient outcomes. What You’ll Do
  • Deliver direct patient care based on the therapist’s plan of care
  • Monitor patient status and report changes requiring reassessment
  • Administer treatment interventions and collect objective data
  • Participate in discharge planning and interdisciplinary case conferences
  • Train patients and caregivers on equipment and therapeutic techniques
  • Maintain accurate documentation and communicate progress to therapists
Qualifications
  • Associate of Arts degree from an approved Physical Therapy Assistant program
  • Minimum 1 year of clinical experience (Home Health or Acute Hospital preferred)
  • Maryland PTA licensure
  • Current BLS (CPR) certification
  • Valid driver’s license and access to a personal vehicle

  • Strong problem-solving and critical thinking
  • Excellent time management and organizational skills
  • Proficient in written and oral communication
  • Ability to use electronic medical records and point-of-care documentation systems
Physical & Mental Requirements
  • Frequent standing, walking, bending, and lifting (up to 50 lbs)
  • Continuous attention to detail and task focus
  • Exposure to blood/body fluids and varying environmental conditions
