Position Goal:

To manage the Quality and Value-Based Care programs in which Hughston Clinic Orthopaedics participates. Focus will be toward ensuring primary total joint arthroplasty (TJA) patients are guided through the entire surgical and recovery experiences to facilitate improved transitions and outcomes.

Position Responsibilities:

  • Manage patient surgical case care for any/all episodes under an alternative payment arrangement program such as, TJR for BPCI and coordinates with physicians and HCO clinical staff.
  • Work with physician lead for Quality Patient Care Programs and assist with communication/meetings between physician lead, HCO physicians and staff, and external care partners.
  • Communicate and train HCO physicians and staff on clinical procedures to include the protocols and processes required by HCO participating programs.
  • Build and maintain relationships with care partners to achieve stated goals.
  • Build and maintain relationships with program initiators, such as payers, employers, and facilities to achieve stated goals.
  • Monitor, analyze, and communicate with HCO physicians and staff regarding internal program data and external program reports (as provided by program initiators).
  • Develop patient communication workflow processes using any new systems for this purpose.
  • Review all program reports, identify trends and process to affect desired program changes, and communicate with physicians and staff regarding reports.
  • Train and orient new clinical staff on programs protocol.
  • Perform pre- and post-surgical telephone calls to assess patient needs and compliance.
  • Conduct comprehensive pre-admission assessments, education and follow-up to ensure services are in place.
  • Follow the patient while hospitalized; escalates long stay/high cost cases and actively engages in potential disposition changes to assure appropriate discharge and follow-up.
  • Utilize current patient communication workflow processes using any new systems for this purpose.
  • Oversee nursing care essential to the assessment, promotion, maintenance, and restoration of patients health and wellness.
  • Participate in professional development activities and maintain professional affiliations to include attending conferences and workshops.
  • Responsible for implementation of OSHA regulations.

Experience:

Required:

  • Two (2) years of clinical experience in a hospital, medical office setting or telephonic case management experience.

Preferred:

  • Experience with discharge planning and outpatient care coordination.
  • Experience with geriatric patients and skilled nursing home facility

Education:

Required:

  • High School Diploma or equivalent.

Preferred:

  • Basic education necessary for Nursing or Athletic Training licensure.

Special Qualifications:

Required:

  • Must have a valid drivers license and satisfactory Motor Vehicle Record (MVR).
  • BLS certification.

Preferred:

  • State nursing or athletic training license.
