Schedule: MondayFriday, 8:00 AM4:30 PM + weekend rotation (1 weekend day every 6 weeks)
No holidays. No night on-call.

$43.30 - $64.95💼 What Youll Do
As a Physical Therapist in our Home Health division, youll:
- Coordinate and deliver medically prescribed physical therapy services with a focus on quality, continuity, and cost-effectiveness .
- Conduct comprehensive evaluations and develop personalized care plans based on musculoskeletal, neuromuscular, cardiopulmonary, and integumentary assessments.
- Monitor patient progress, adjust treatment plans, and communicate updates to physicians and leadership.
- Advocate for patients by managing their medical conditions, medications, and functional goals.
- Supervise Physical Therapist Assistants (PTAs) and collaborate with Home Health Aides to maximize patient outcomes.
- Educate patients and caregivers on treatment techniques and adaptive equipment.
- Participate in chart audits, quality improvement initiatives, and discharge planning.
- Mentor new employees and serve as a clinical instructor for students.
🧾 What Youll Need
- Licensure: Physical Therapist license (Wisconsin)
- Certifications: BLS for Healthcare Providers (AHA) must be obtained within 6 months
- Education: Bachelors Degree in Physical Therapy
- Experience: Minimum 3 years in acute hospital, home health, or rehab setting
- Skills: Strong clinical judgment, communication, organization, and decision-making abilities
- Tech: Basic proficiency in Microsoft Office (Word, Excel, PowerPoint)
- Transportation: Valid drivers license and insured, functional vehicle
