Schedule: Exempt, min 40 hours per week | 8am - 4:30pm | Monday - Friday

What You’ll Do in This Role:

As an Occupational Therapist, you will evaluate participants’ functional abilities, create and carry out individualized treatment plans, and provide interventions that enhance independence, mobility, and safety. You’ll recommend adaptive equipment or home modifications when needed, educate participants and caregivers, and work closely with the interdisciplinary team to ensure coordinated, person-centered care.

Primary Responsibilities:

  • Conduct initial and ongoing assessments to evaluate participants’ rehabilitative needs.
  • Identify participant goals and develop, implement, and revise individualized care plans.
  • Assess the need for adaptive equipment or durable medical devices and recommend to the IDT for approval.
  • Plan and deliver personalized therapy programs, including exercises, treatments, modalities, and assistive devices.
  • Collaborate with Recreational Therapy to adapt activities or design alternative exercise programs.
  • Provide therapy in a variety of settings, including participants’ homes, the community, and the LifeCircles Center.
  • Support discharge planning from hospitals, assisted living, or skilled nursing facilities to ensure continuity of care.
  • Follow safety protocols and ensure all care is delivered effectively and in compliance with guidelines.
  • Provide training and education to caregivers and family members to support participants at home and in the community.
  • Actively participate in IDT meetings, communicate changes, update care plans, and track progress toward goals.

What You’ll Need:

  • Must be licensed to practice as an Occupational Therapist in the State of Michigan with an unencumbered, active license.
  • Knowledge of current Occupational Therapy practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • Strong critical thinking, creativity, flexibility, and problem-solving skills.
  • Ability to work effectively as part of a multidisciplinary team.
  • Proficiency in computer use, including Microsoft Excel, Office, Outlook, and electronic medical records.
  • CPR or will to obtain with 30 days of hire.
  • Ability to move and assist participants, including the use of equipment such as wheelchairs, walkers, and other assistive devices.
  • Ability to work in a variety of settings, including participants' homes and the PACE center.
