Join our rehab team at The Meadows in Norfolk, NE as a PRN / On-Call Physical Therapist! In this flexible role, youll provide therapy services on an as-needed basishelping residents regain strength, mobility, and independence. Location: The Meadows Norfolk, Nebraska (part of Nye Health Services)
Key Responsibilities
  • Evaluate residents and develop personalized treatment plans to address functional deficits, mobility needs, and rehabilitation goals.
  • Provide direct therapy services in accordance with physician orders and industry standards.
  • Collaborate with an interdisciplinary teamincluding nurses, physicians, and other therapiststo ensure holistic resident care.
  • Educate residents, families, and caregivers on therapeutic techniques and strategies to promote independence and safe mobility.
  • Accurately document evaluations, progress notes, and treatment outcomes in compliance with regulatory and facility requirements.
Qualifications
  • Current Physical Therapist license in the state of Nebraska (or eligibility).
  • Degree in Physical Therapy from an accredited program.
  • Strong clinical and communication skills, with the ability to motivate and encourage residents.
  • Previous experience in a skilled nursing, assisted living, or rehabilitation setting preferred.
