Your daily responsibilities will encompass evaluating patients in both inpatient and outpatient settings, designing personalized treatment plans, and administering physical therapy interventions that prioritize mobility, strength, balance, and overall function. You will collaborate closely with nurses, physicians, case managers, and fellow rehabilitation professionals to maximize patient outcomes.

Education and communication are integral to your role. Youll be educating patients and their families about their therapeutic goals, discharge plans, and essential home exercises. You will meticulously document evaluations and treatment progress in line with our facilitys standards and participate in interdisciplinary conferences to promote coordinated care.

To thrive in this position, you are required to have a Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program and maintain state licensure as a Physical Therapist, with CPR certification being preferred. Specialized certifications, while beneficial, can be pursued after joining us.

With prior experience being highly regarded, you must also demonstrate the ability to treat patients with complex medical and mobility issues, coupled with exceptional communication and teamwork skills. Proficiency in digital charting and clinical documentation is necessary to streamline processes and enhance patient care.
