Job DescriptionGeneral Purpose of Job:
Provides physical therapy treatment to assigned patients in accordance with the treatment plan of care. Assumes responsibility for creating experienced-based service.
Essential Duties:
- Consistently exemplifies on-stage behavior.
- Is responsible for creating a destination, experience-based service; delivers customized experiences using patient profiles.
- Provides physical therapy treatment to assigned patients in accordance with the treatment plan of care provided by physical therapist.
- Instruct patients in safe transfers, activities of daily living, correct ambulation, therapeutic exercises and correct usage of exercise equipment; trains family members and caregivers on various therapeutic procedures and home exercises.
- Performs physical therapy screenings, including range of motion measurements, assesses patient progress; recommends changes to the plan of care as appropriate.
- Maintains communication with the Physical Therapist, nurses and physicians to facilitate quality patient care; reports abnormal physiological changes.
- Documents patient treatments, progress notes and discharge summaries in a timely manner and in accordance with Hospital policies and insurance requirements.
- Assists in developing, marketing and implementing various Rehab Services programs including, but not limited to, Aquatic Physical Therapy Program and the Encore program.
- Schedules patient treatments/appointments along with referring patients to other hospital services.
- Provides guidance and training to appropriate staff regarding home exercise programs.
- Attends staff meetings and in-service programs as required.
- Assists in cleaning and maintaining therapy equipment and treatment areas; notifies appropriate personnel of supplies needed to be ordered.
- Supports and serves as a role model for our mission, vision, values, and customer service initiatives. Adheres to the organizations policies & procedures, and compliance guidelines.
- Performs other duties as assigned.
