**About the Role**

Were seeking a passionate and skilled **Occupational Therapist** to join our Home Health team. In this role, youll provide in-home evaluation and therapy for clients with physical, emotional, or cognitive limitations. Youll help patients achieve greater independence through customized care plans, family education, and goal-oriented therapy interventions.

**What Youll Do**

+ Conduct home-based assessments within 72 hours of referral (unless otherwise specified).

+ Design and implement individualized treatment plans focused on improving daily living skills and function.

+ Educate patients and caregivers on therapy techniques, home exercise programs, and insurance guidelines.

+ Participate in interdisciplinary case conferences and collaborate across care teams.

+ Maintain clear, accurate documentation in compliance with all home health guidelines and regulations.

+ Ensure care is age-appropriate across the lifespan, from infants to geriatrics.

**What Youll Need**

+ **Bachelors degree in Occupational Therapy required** (Masters preferred).

+ **Current Iowa Occupational Therapy License** (provisional license with supervision accepted for new grads).

+ **Valid Iowa drivers license** , proof of insurance, and reliable transportation.

+ BLS certification and Mandatory Reporter certification.

+ 02 years of experience in occupational therapy (home health experience preferred).

+ Strong communication, organization, and independent problem-solving skills.
