As a Physical Therapist specializing in outpatient rehabilitation, you will help individuals of all ages and abilities improve their mobility, function, and overall quality of life. You will work closely with a collaborative team of therapists and assistants, treating a diverse range of conditions and benefiting from a supportive environment that prioritizes mentorship and professional development. *Key Responsibilities:*
* Perform comprehensive patient evaluations and design customized treatment plans using a wide range of therapeutic interventions.
* Apply knowledge of growth and development principles to provide care throughout the patient lifespan.
* Monitor and adjust treatment plans to help patients achieve maximum functional independence.
* Coordinate and communicate effectively with other healthcare disciplines and services to support patient goals.
* Advocate for patients and their caregivers by providing education and connecting them with community resources.
* Contribute to discharge planning and ensure smooth transitions of care.
* Maintain accurate documentation using electronic health record systems.
* Ensure compliance with practice standards, regulations, and safety protocols.
* Provide care in various settings, which may include outpatient clinics, inpatient units, long-term care, community-based care, and home health.
* Perform other duties as assigned to support patient care and team goals.
*Requirements:*
* Bachelor's degree in Physical Therapy (required).
* Current Physical Therapy license in the state of New Mexico (required).
* Basic Life Support (BLS) certification at the time of hire (required).
* Basic computer proficiency.
* Understanding of clinical rules and regulations.
* Less than one year of experience in physical therapy; new graduates are encouraged to apply.
*Preferred Qualifications:*
* Doctor of Physical Therapy degree.
