GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
The following is a list of job duties and responsibilities, including essential and marginal test:
Responsible for providing mental health services to severely and chronically, mentally ill or emotionally disturbed individuals (adults and/or children) and their families, to include dual disordered mental health/chemically dependent individuals. Provides ongoing case management services to assist assigned individuals in gaining access to medical, social, educational and other needed services. Service interventions are provided in home, school and community settings, and include: screening, admission to service, consumer assessments, treatment planning, team treatment staffing, continuity of services monitoring, referral and follow up services, individual Rehabilitative services, family, rehab has skills, training, group, skills, and provision of crisis resolution services. Performs the duties as addressed in the Texas administrative code, community standards, and policies and procedures of the agency. Required to adhere to the agency productivity standards.
Assist the director in providing community education and consultation. Interagency collaboration is essential to this position. The job requires extensive travel to community-based service, provision and flexible hours and schedule. Participates in on-call system if a QMHP - CS and received required clinical supervision by appropriate staff. Administratively reports directly to the Clinic Director.
MINIMUM JOB QUALIFICATIONS:
Applicants must meet the following minimum qualifications to be eligible for the position:
QMHP - CS case manager:
Bachelor's degree from an accredited college or university with a major and social, behavioral, or human services
Five (2) years of full-time work experience in human services case management
All QMHP positions must meet Qualified Mental Health Professional (QMHP) requirements
Minimum of 18 hours graduate coursework in a related field may substitute for one year of work experience
PREFERRED JOB QUALIFICATIONS:
Applicants who possess one or more of the following qualifications in addition to the minimum qualifications are preferred
KNOWLEDGE, SKILLS, AND ABILITIES:
Ability to read, write and speak English
Ability to work under conditions requiring physical stamina, and ability to work in a stressful environment
Ability to complete documentation as required and report, accurately and timely
Ability to complete the required training
Skilled in customer relations with strong professional communication skills
Works well with coworkers and teams
Must possess good computer skills
Must possess a valid driver's license with an acceptable driving record
