As an Occupational Therapist (OT) in our Home Health team, youll bring your clinical expertise and compassionate care directly to patients in the comfort of their own homes. You'll play a key role in developing and delivering personalized therapy plans that help patients regain independence, improve their quality of life, and achieve their goals. Working closely with physicians and the care team, youll provide hands-on treatment, education, and supportall grounded in best clinical practices. ResponsibilitiesMakes visits to the homes of patients requiring home health occupational therapy services and assumes responsibility for an ongoing interdisciplinary assessment of the patient.
Evaluates and assesses the patients level of functioning by applying appropriate tests.
Develops specific occupational therapy procedures as well as functional activities in accordance with written physician plan of treatment and patient evaluation.
Develops and administers plan of treatment including environmental modifications.
Oversees use of treatment modalities, prosthetic devices and therapeutic appliances.
QualificationsEducation:
- Graduate of an occupational Therapist education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or successor organizations of ACOTE
- Eligible to take, or has successfully completed the entry-level certification examination for Occupational Therapists developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT)
Licensure/Certifications:
- Licensed or otherwise regulated, if applicable, as an Occupational Therapist by the state in which practicing, unless licensure does not apply in the practicing state.
- Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
- Mandatory Reporter: Child & Dependent Adult Abuse.
- CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
Minimum Experience:
- One year experience caring for similar patient population as to be assigned.

- Use of usual and customary equipment used to perform essential functions of the position
