Responsibilities
Provide multi-stage individual treatment in the community and group treatment in the office when necessary
Develop individualized service plan (s)
Help coordinate scheduling of other treatment services such as therapy, medication services and promoting interest and concern for outside medical appointments
Meet billable productivity expectation set by the agency
Complete documentation in accordance with agency policy
Consult with community agencies and families
Gather client medical records
Contact the client's family/significant others in case of emergencies
Maintain communication with referral sources in timely manner and coordinate case management plans
Document client progress and contacts in timely manner
Participate in therapeutic and psycho-educational interventions
Conduct home visits
Work with client to ensure medication needs are met.
Provide community linkage to appointments as needed
Reach out and provide assistance when the client exhibits non-compliance or a poor response to interventions
Participate in staff organizational meetings and treatment planning review meetings
Participate as a treatment team member in updating progress
Could be required to provide crisis intervention
Abides by the company Always Behaviors each and every day as we serve our clients, colleagues, and community.
All other duties as assigned and/or appropriate to the position.
Qualifications
Education and Licenses or required certifications: Bachelor's degree or High School Diploma with 3 years case management experience required as allowed by regulatory body. Valid driver's license and auto insurance. Must have the ability to maintain active insurance and driver's license. CDCA or willingness to obtain CDCA upon hire.
Experience: Prefer clinical skills with experience in an inpatient or outpatient setting and knowledge and experience with electronic health records. Prefer prior experience documenting in an electronic health record.
Specialized knowledge, skills, or abilities: Must have a strong commitment to client rights. Must have skills and competence to establish supportive and trusting relationships with clients. Knowledge and skilled regarding clients with co-occurring disorders.
