LOCATION: Dublin, GA

The Community Service Board of Middle Georgia's Step One department is seeking a dynamic and resourceful individual to serve as our next CST (Community Support Team) Case Manager! Reporting to the Sr. Manager, Behavioral Health, our CST Case Manager will have a focus on working with individuals to increase progress in relation to their behavioral health.

Responsibilities of the Secretary:

  • Monitor and document consumer progress. Per standards for CST, make required contacts per month to consumers
  • Adhere to treatment plans and provide services and interventions as planned. Write progress notes (24-48 hrs.) and bill for services provided
  • Adhere to DHR guidelines in regard to the quality of documentation
  • Recognize consumer needs and assist them in obtaining appropriate services
  • Provide consumer information for outside resources and encourage them to access them as independently as possible
  • Provides 24/7 coverage as needed for the CST individuals
  • Attends and participates in meetings pertaining to case management, case collaboration and discharge planning of assigned consumer
  • Works toward providing 60% of the team’s services in the community and face to face; increasing consumer engagement
  • Provides information sharing between case management staff and others (i.e., residential staff, etc.) on mutually served consumers
  • Effectively advocates for consumers when identified needs cannot be met within existing services
  • Promotes consumer self-determination and decision making and provides essential expertise and consultation to the entire team to promote a culture in which clients point of view and preferences are recognized, understood, respected and integrated into treatment, rehabilitation, medical and community self-help activities
  • Transports or arranges transportation for consumers as needed
  • Performs all additional duties as assigned

  • High School Diploma/GED Equivalent
  • Excellent interpersonal and communication skills with a high level of professionalism and the ability to work effectively with diverse populations
  • Valid Georgia Driver’s License
  • Ability to work independently and in collaboration with others
  • Experience with Microsoft 365 Office Products
