Position Summary

This is a telework/work at home position with up to 20% travel throughout the South Florida/Miami Dade and Monroe County areas. Standard business hours Monday-Friday.

This Clinical Case Manager Behavioral Health position will focus on pediatric and adult populations. The Clinical Case Manger BH coordinates all case management activities with members to evaluate behavioral health needs and to facilitate the overall wellness of members. Develops strategies to address issues to outcomes and opportunities to enhance member's overall wellness through integration. Instructs programs and procedures in compliance with network management and clinical coverage policies. The Clinical Case Manager BH will coordinate with members, family, clinical services, treatment team and supervisors on the development, planning and implantation of therapeutic services as they relate to the individual's behavioral and emotional health.

Essential Duties and Responsibilities:

Care Planning and coordination of needs

Provides services consistent with the guiding principles of the agencys philosophy of strength based, family focused, needs driven and culturally competent care.

Completes assessments, formulate, and implement an overall comprehensive care plan for each member, consistent with the needs of the member, team and the plan.

Gather information and develop a comprehensive care plan that addresses risk factors, psychosocial history, substance abuse history, mental health history, community functioning, performance outcomes, strengths, challenges, goals and interventions.

Identify potential problems and crisis areas, prioritize according to seriousness, and create an effective and well specified safety plan.

Collaborate with members, family, clinical team members, DCF, external providers, and natural supports for case conceptualization, assessment, and treatment planning.

Required Qualifications

  • Active and unrestricted independent clinical Behavioral Health licensure in Florida: LCSW, LMHC, LMFT
  • Minimum 2 years of clinical experience post masters degree.
  • Minimum 2 years of experience working with children and/or adults in a clinical or behavioral health environment.
  • Willing and able to travel up to 20% of their time to meet members face to face throughout South Florida including Miami-Dade and Monroe counties.
  • Reliable transportation required; Mileage is reimbursed per our company expense reimbursement policy.
  • 2+ years experience using personal computer, keyboard navigation, navigating multiple systems and applications; and using MS Office Suite applications (Teams, Outlook, Word, Excel, etc.)

Preferred Qualifications

  • Managed Care experience
  • Case Management experience
  • Certified Clinical Case Manager
  • Bilingual: Spanish or Creole

Education

  • Masters Degree in Social Work, Counseling or related field is required.

Pay Range

The typical pay range for this role is:

$54,095.00 - $116,760.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
