Description
  • Conduct assessments of patients in both inpatient and outpatient environments and create personalized treatment plans
  • Implement physical therapy interventions that prioritize mobility, strength, balance, and functional capabilities
  • Coordinate with nurses, doctors, case managers, and rehab specialists to promote optimal patient outcomes
  • Provide education to patients and their families about therapeutic objectives, discharge plans, and ongoing home exercises
  • Maintain detailed documentation of evaluations, treatments, and progress in line with facility and regulatory standards
  • Engage in interdisciplinary team meetings and care rounds to ensure cohesive care delivery
Education
  • Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
Certifications
  • State licensure as a Physical Therapist
  • CPR certification is highly recommended
  • Additional specialized certifications (e.g. neurological, orthopedic) advantageous but not mandatory
Skills
  • Prior experience strongly preferred
