Under the general direction of the Clinical Program Manager, requires independent judgment in the development and implementation of services, plans, and coordinates community-based services to children and their families. Job performance contributes to achieving the goals of San Benito HHSA services (FURS, Home Visitation, and CFT Facilitation) to enhance the well-being of families in the community. Implementing typical responsibilities and duties requires performing job duties, explaining and instructing others, persuading others, and negotiating with others on job-related matters of moderate complexity.
TYPICAL RESPONSIBILITIES AND DUTIES:
Home Visitation:
- Meet with clients and their families face-to-face to provide the appropriate level of services.
- Gather pertinent information through the administration of tools (ASQs) to complete the Family-Centered Assessment.
- Provides quality, culturally proficient, trauma-informed, and relevant services to families and their children under 6 at home and in the community.
- Provides goal-focused brief intervention and monitors client and family progress.
- Provides parenting support services, including individual and group, to families and their children with developmental and behavioral concerns.
- Demonstrates the ability to work with communities, workgroups, and serve families with diverse economic, social, racial, and cultural backgrounds.
- Engages in training, provides education, and specialized intervention services to clients and their families involved in Home Visitation services using best practice, promising practices, or evidence-based models.
- Ensures services are delivered true to fidelity by engaging and participating in trainings and supervisions (individual and group).
- Documents necessary information as required within a timely manner and consistently meets or exceeds agency productivity standards and completes coordinated documents such as treatment plans and assessments prior to their due date.
- Maintains professional standards regarding client rights.
Child and Family Team (CFT) Facilitation:
- Facilitate CFT Meetings for children and families in services with San Benito County HHSA.
- Schedule and coordinate location and meeting times with CFT members.
- Demonstrate the ability to work as part of a coordinated-care team in a manner that aligns with State standards.
- Ensures documentation is accurate and submitted within the timeframe outlined by SBC HHSA.
Family Urgent Response Services (FURS):
- Provides follow-up services to children and families after a crisis, including reviewing and modifying safety plans.
- Refers and ensures children and families are connected to additional services that can support them in addressing stressors.
- Ensures documentation is accurate and submitted within the timeframe outlined by SBC HHSA.
- Registers to participate in the FURS on-call system.
- Demonstrates an understanding of confidentiality policies, mandated child abuse reporting, and duty to warn procedures. Utilizes clinical supervision in any case where these issues are in question.
- Every employee has the opportunity and responsibility to participate in one or more activities each year that support the performance quality improvement (PQI) of RCS programs and services. Activities might involve membership on a PQI committee or work group, service as a peer reviewer, data collection for PQI and evaluation purposes, completion of PQI questionnaires and surveys or participation in focus groups, or other activities as identified by PQI committees or work groups.
EDUCATION, TRAINING AND EXPERIENCE:
- Associate or bachelor’s degree in social services, human services or related (or 4 years of equivalent years of experience) preferred.
- Must meet the Title 9, Title 19, and Title 22 Medi-Cal Regulations
- Experience with early childhood development and behaviors, relationship based and parent/child practices/models.
- Experience working in children’s mental health setting emphasizing a family systems perspective desirable.
- Basic skills in crisis intervention, domestic violence, suicide assessment, conflict resolution, and interpersonal communication.
- Bilingual/ Bicultural (Spanish/English) and/or other threshold languages preferred.
LICENSE(S):
- California driver’s license and ability to be insured by RCS insurance provider.
KNOWLEDGE AND SKILLS:
- Ability to maintain a positive perspective in the treatment and care of emotionally disturbed youth.
- Ability to follow directions and to work as part of a team.
- Ability to develop and maintain a creative and optimistic spirit.
SPECIAL REQUIREMENTS:
- Must be able to meet and receive a criminal records clearance as required by Title XXII licensing regulations.
PHYSICAL AND MENTAL REQUIREMENTS:
The following are required in day-to-day performance of the duties of this position:
- Sitting, walking, standing, grasping (simple), and visual/auditory acuity are constantly required (over 2/3 of the workday).
- Bending, squatting, kneeling, twisting, lifting, reaching, and pushing/pulling are required occasionally (under 1/3 of the workday).
- Climbing and crawling are generally not required.
- Community-Based travel is required.
