Physical Therapist - Rehab

We are seeking a compassionate and skilled Physical Therapist to join our rehabilitation team. This role involves evaluating, planning, and implementing individualized treatment programs to help patients restore function, improve mobility, relieve pain, and prevent disability. The ideal candidate will have strong clinical judgment, excellent communication skills, and a patient-centered approach to care.

Key Responsibilities
  • Perform comprehensive patient evaluations, including medical history review, physical examination, and functional assessments.

  • Develop and implement individualized treatment plans in collaboration with patients, families, and interdisciplinary team members.

  • Provide therapeutic exercises, manual therapy, gait training, and other evidence-based interventions to address functional limitations.

  • Educate patients and caregivers on treatment plans, home exercise programs, and strategies to prevent further injury.

  • Monitor and document patient progress, adjusting treatment plans as needed to achieve optimal outcomes.

  • Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated patient care.

  • Maintain accurate, timely, and compliant documentation in the electronic medical record system.

  • Adhere to all facility policies, procedures, and safety guidelines.

  • Stay current with industry trends, research, and best practices in physical therapy and rehabilitation.

Qualifications
  • Education: Doctorate or Masters degree in Physical Therapy from an accredited program.

  • Licensure: Current and valid Physical Therapist license in Texas.

  • Experience:

    • Preferred: 1+ years of experience in a rehab, inpatient, outpatient, or skilled nursing setting.

    • New graduates with strong clinical skills will be considered.

  • Certifications: CPR/BLS certification required. Additional rehab-related certifications a plus (e.g., Neurological Rehab, Orthopedic Rehab).

  • Strong interpersonal, organizational, and problem-solving skills.

  • Ability to work independently and as part of a multidisciplinary team.

Work Environment
  • May work in various settings including inpatient rehabilitation, outpatient clinics, or skilled nursing facilities.

  • Requires standing, walking, lifting, and assisting patients during therapy sessions.
