Shift: MondayFriday, 56 patients per day (approx. 27/week)

Job Description:
Sunbelt Staffing is hiring a motivated Home Health Physical Therapist for a 13-week travel contract in Longmont, CO, with the potential to move into a permanent position. This role involves providing high-quality therapy services to patients in their homes, managing around 27 visits per week, and ensuring timely documentation24 hours for SOC and 48 hours for all other visits. The position uses MatrixCare, and orientation includes 1 day of shadowing plus additional training if needed.

Key Responsibilities:

• Deliver physical therapy services in accordance with physician orders and care plans

• Complete start-of-care, routine, and discharge visits efficiently and accurately

• Document all visits within required timeframes using MatrixCare

• Educate patients and caregivers on therapy techniques and goals

• Coordinate with the home health team to promote optimal patient outcomes

Qualifications:

• Active Colorado PT license

• Current BLS certification

• At least 6 months of home health experience preferred

• Experience with MatrixCare required

• Valid drivers license, dependable transportation, and proof of insurance

• Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
