PAY AND BENEFITS
Annual Pay Range: $61,374.91 - $77,335.94
Hourly Pay Range: $29.507168 - $37.180740
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.
- Bilingual Spanish/English skills are required for this position. An additional 5% of base compensation is added to pay.
Health Centers-Behavioral Health provides licensure supervision to all regular status employees who have completed a Graduate degree in a behavioral health related field (social work, counseling, marriage and family therapy, etc.) and are actively pursuing a licensure in that field.
Group supervision is available for CADC hours for those on a CADC track.
Clackamas County's Health Centers are Federally Qualified Health Centers.
There may be options for loan forgiveness while working with a government / public agency through programs such as the National Health Service Corps (NHSC) Loan Repayment Program or the Public Services Loan Forgiveness (PSLF) program.

JOB DETAILS AND QUALIFICATIONS
Clackamas County Health Centers is seeking a Bilingual (Spanish) Case Manager to support the Community Support Team within our Behavioral Health Clinics at our Milwaukie Lake Road Clinic. This is an excellent opportunity to make a meaningful impact by helping individuals experiencing mental health and addiction challenges access the support and care they need to live healthier lives. As part of a multidisciplinary and compassionate team, you'll play a key role in strengthening our community through direct care and service coordination.
The Case Manager provides comprehensive care coordination and case management services to adults living with mental health conditions and/or substance use disorders, many of whom experience severe and persistent symptoms. Responsibilities include skills training, medication education and support, resource navigation, and connecting clients to appropriate treatment and recovery services. The role involves working collaboratively with families and community partners to support individuals through individual, group, and family-based interventions as outlined in the treatment plan. Case Managers may also provide targeted support to clients with co-occurring health conditions such as diabetes. Services are delivered across various settings including clinics, homes, hospitals, care facilities, and community locations, and require the ability to effectively engage a diverse client base while maintaining professionalism in high-need environments. This position must meet the standards of a Qualified Mental Health Associate (QMHA) as defined by the Oregon Administrative Rules.

Required Minimum Qualifications/ Transferrable Skills:*
- Qualified Mental Health Associate (QMHA) or (QMHA- R) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO) as mandated by Oregon Administrative Rule at the time of hire.
A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:
(a) A Bachelor's degree in a behavioral sciences field; or
(b) A combination of at least three (3) years relevant work, education, training or experience.
- A minimum of three (3) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position
- Fluency in Spanish/English, both oral and written (bilingual skills to be validated/tested at time of interview and/or selection)
- Must possess and maintain a Basic Life Support (BLS) certificate
- Experience providing case management services to clients diagnosed with diabetes, severe and persistent mental illness (SPMI), and/or substance use disorders (SUD)
Preferred Special Qualifications/ Transferrable Skills:*
- Experience working in a primary care or behavioral health care setting
- Experience providing services and/or support to vulnerable clients
- Experience working with Evidence Based Treatment Practices

TYPICAL TASKS
Duties may include but are not limited to the following:
- Provide group, individual, and family services as appropriate and identified in the Primary Care and/or Mental Health treatment plan: Actively engage clients in the most appropriate setting (home, office, community). Provide outreach to clients for missed appointments. Utilize agency-approved Evidence-Based Practices. Utilize outcome measures identified by Clackamas County Behavioral Health Centers (CCBHC) to monitor client progress in their recovery. Make mandatory reports as defined by state law and agency policy.
- Provide case management, skills training, and care coordination services for individuals who are diagnosed with SPMI and diabetes. Provide outreach as indicated in the client's care plan. Develop and coordinate resources to provide clients and potential clients with needed services, community outreach, and coordination with service providers.
- Coordinate with health care providers, social service personnel, and other agencies to obtain support and services; provide information and referral services to clients and families; reevaluate and update case plans and client eligibility on a regular basis; advocate for clients and other agencies for services.
- Provide individual/family skills training as appropriate and identified in the treatment plan on the following activities of daily living: safety planning, health practices, communication, time management, money management, nutrition, mental health symptoms, medication management, problem solving, family relationships, alcohol/drug use, leisure, community resources, social network, sexuality, productivity, coping skills, behavior norms, personal hygiene, grooming, and dress.
- Determine health care and social service needs of clients; interview applicants, recipients, relatives, guardians, care providers, and other involved parties; conduct comprehensive needs assessments, including psychosocial, financial, and physical factors; evaluate functional impairment of clients.
- Provide other duties as assigned.
WORK SCHEDULE
This position works 40 hours during a standard workweek of Monday through Friday.
Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
This position is eligible for hybrid on-site/telework subject to the Clackamas County Teleworking Policy and based on the Department's business needs. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
