Manageable Caseloads - Quality-focused approach to ensure optimal patient care. What You Will Do: Provide nursing case management services for individuals with behavioral health needs, ensuring continuity of care. Conduct health assessments and develop individualized care plans. Collaborate with interdisciplinary team members to coordinate treatment and support services. Monitor patient progress, adjust care plans as necessary, and ensure documentation is accurate and timely. Educate patients and families about treatment plans, medications, and self-care strategies. Facilitate referrals and linkages to community resources. Who You Are: Registered Nurse with 1+ years of experienc e licensed in Massachusetts (required) Strong assessment, documentation, and communication skills. Reliable transportation and willingness to travel within the service area. Must be at least 21 We Hire for Purpose!
