Speech Language Pathologist PACE Program
Join our mission-driven team at Trinity Health PACE, where youll play a vital role in enhancing the lives of older adults. As a Speech Language Pathologist, you will evaluate and treat participants with speech, language, cognitive-communication, voice, fluency, and swallowing disorders. Your work will support participants in achieving their highest level of independence and quality of life.
Key Responsibilities:
- Provide assessments and individualized treatment plans following Interdisciplinary Team approval.
- Recommend safe swallowing strategies and appropriate diets.
- Document care in TruChart and maintain accurate records.
- Collaborate with team members to develop participant-centered care plans.
- Ensure compliance with all regulatory and ethical standards.
- Participate in training, team meetings, and quality improvement initiatives.
Qualifications:
- Masters degree in Speech Language Pathology or related field.
- Current, active, and unencumbered SLP license in the state of practice.
- ASHA certification and CPR certification required.
- Strong communication, organizational, and interpersonal skills.
- Proficiency in Microsoft Office and electronic documentation systems.
