Job Details Job Location : FHN Memorial Hospital - FREEPORT, IL Position Type : Full-time; F (80) Salary Range : $29.99 - $49.48 Hourly Job Shift : Days Job Category : Health Care Description Job Summary Specializes in diagnosis and treatment of speech and language problems for patients. Also engages in scientific study of human communication by performing the following duties. Job Responsibilities Assists the physician in determining and recommending appropriate speech and language services, necessary adaptive equipment, and swallow precautions/diet recommendations. Evaluates and diagnoses speech and language skills as related to educational, medical, social, and psychological factors. Evaluates and diagnoses swallow ability for safe eating/drinking. Develops, plans, implements, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and/or swallowing problems of organic and nonorganic etiology, individualized to each client's needs interests and abilities. Provides counseling, guidance and language development therapy to individuals with disabilities. Administers, scores, and interprets specialized communication, speech and language tests. Records and reports to the client's physician the client's progress in treatment and any changes in the client's condition and plan of care. Participates in process improvement/quality indicator tracking. Completes relevant documentation and record keeping of stated services and consultation with other disciplines and services, including but not limited to, evaluations, daily progress and progress notes, in the appropriate time frame. Maintains records as required by law, establishment's policy, and administrative regulations. Evaluates and monitors individuals. Reviews treatment plan, and assesses individual performance to modify, change, or write new programs. Instructs individuals to monitor their own speech and provides ways to practice new skills. Acts as consultant to educational, medical, and other professional groups. Provides education and training for patients and families. Attends and participates in customer care, staff conferences and other meetings. Displays competency for age specific patients; pediatrics through geriatrics. Displays competency for the Speech Language Pathologist role as evidenced in the annual performance appraisal following the FHN PRIDE Standards. Performs video fluoroscopic swallow testing in cooperation with Radiology department. Supervises Rehab tech, students and volunteers. Other duties may be assigned. Qualifications Requirements Education: Master's Degree from accredited College or University Required Experience : 2 - 5 years related experience and/or training is preferred Licensure/Certification/Registration: Current licensure as a Speech Language Pathologist in Illinois Maintain Certificate of Clinical Competency (CCC) through the American Speech Language Hearing Association CFY progress is required Maintain AED/BLS certification Physical Requirements: Activity Never Rarely Occasionally Frequently Constantly Bending/Stooping X Climbing X Keyboard Data Entry X Kneeling X Reaching X Repetitive Foot/Leg Movements X Repetitive Hand/Arm Movements X Running X Sitting X Squatting X Standing X Walking X Lifting/Carrying (Non-Patient) 0 - 25 lbs.
