• Conduct evaluations for patients in either inpatient or outpatient settings, devising personalized treatment plans based on individual needs
• Utilize specialized physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Work closely with nurses, physicians, case managers, and fellow rehabilitation professionals to maximize patient outcomes
• Guide patients and their families through therapeutic objectives, discharge processes, and home exercise regimens
• Maintain meticulous documentation of evaluations, treatments, and progress in line with facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure cohesive patient care

• Hold a Doctorate in Physical Therapy (DPT) or an equivalent accredited degree

• Possess state licensure as a Physical Therapist
• CPR certification preferred
• Additional certifications (e.g., in geriatrics or neurological conditions) are welcomed but not mandatory

• Prior experience is strongly preferred
• Able to treat patients with various complex medical conditions and mobility challenges
