(Salary range from $75,000 to $120,00 per year)

What You'll Do

* Serve as a seasoned social work consultant in long-term care facilities

* Provide individual and group psychotherapy sessions

* Complete mood and symptom-based screenings

* Develop individualized treatment plans for adults and/or seniors

* Provide in-service education

* Collaborate with facility staff and consult with interdisciplinary teams

* Maintain accurate, compliant documentation

Qualifications

* Master's Degree in Social Work

* Licensed Mental Health Counselor with the ability to practice and bill independently

* Active license to practice in State(s) where MediTelecare operates

* Significant experience in treating adult and/or senior patient population, particularly with patients exhibiting severe persistent mental illness

* Strong communication skills and cultural humility
