Summary: The Occupational Therapist is responsible for the coordination of O.T. programs on the Resident units. Performs professional, clinical, and overview duties to assure program meets the needs of individual residents and interfaces with unit program and facility and resident goals.
YOUR EXPERIENCE COUNTS: Each additional year of approved formal education may be substituted for one year of required work experience and/or each additional year of approved work experience may be substituted for one year of required formal education.
MINIMUM QUALIFICATIONS:
Education/Experience:
Bachelor's degree and 1 year of experience OR equivalent combination of 5 years of education and experience after completion of high school. Education and experience must be in occupational therapy or related field.
License/Certification:
Licensure as an Occupational Therapist by the NH Allied Health Professional Governing Board for Occupational Therapy
Certification by the American Occupational Therapy Certification Board, Inc.
Other Requirements:
None
Trainee Option:
Candidates with an equivalent combination of 4+ years of relevant education and experience after completion of high school may be considered and hired in 'trainee status' for this position.
PREFERRED QUALIFICATIONS:
Experience in Long Term Care settings working with the Elderly
Responsibilities:
Administers Initial Screening Assessments and Occupational Therapy Assessments by collecting, summarizing, and interpreting data to determine functioning level of resident.
Develops, implements and monitors individualized Occupational Therapy treatment programs to maximize resident functioning in social, emotional, cognitive, physical, and wellness areas. Interventions are carried out via individual and/or group programs. Completes other treatment duties as assigned by supervisor.
Documents resident progress in the medical record; findings of assessment, treatment plan and progress are entered into medical records according to policy.
Collaborate with multidisciplinary team members, Rehabilitation Service disciplines and community agencies verbally and in writing, to ensure effective interdisciplinary resident services and continuation of services post discharge.
Develops and monitors implementation of comprehensive treatment programs to meet overall and individual resident goals and objectives.
Trains and guides assigned staff in clinical and technical aspects of duties to assure quality services are developed to meet age and disability needs of population.
Performs Quality Improvement and Appropriateness activities to monitor and improve aspects of care.
Maintains clinical treatment areas and equipment free of hazardous elements and in optimal therapeutic conditions and assures that supplies are sufficient to meet resident and program needs.
Participates in education activities relevant to age and disability needs of population served to keep current on new knowledge in the field and licensure.
