$44.15 - $66.23 an hour - Per Diem Days, Various shifts and hours per week
*Range includes per diem differential
Boulder Community Health is seeking a Physical Therapist - Outpatient Rehab Services who assumes responsibility and accountability for providing quality patient care through the provision of physical therapy services including: assessment of patient functioning, development of treatment programs, provision of patient treatment, and education of the patient and family as it relates to physical/functional abilities; is responsible for working at no less than a 75% productivity rate; must adhere to the APTA Code of Ethics; is responsible for student supervison and aide supervision in the delivery of patient care.

Qualifications
•Current Colorado state license for Physical Therapy.
•BLS certification required by the end of conditional period.
Responsibilities
•Selects appropriate tests and measures to determine goals and problems, course of treatment and appropriate therapeutic procedures/modalities.
•Selects and establishes physical therapy diagnosis.
•Formulates and maintains an individualized patient plan of care based on functional long and short term goals.
•Provides treatment using appropriate tools and demonstrates a complete knowledge of patient management.
•Evaluates patient response to treatment and revises plan of care as indicated.
•Promotes effective communication and teamwork with team members, the hospital and other contacts.
•Reports and documents evaluations, plans, treatment techniques, assessment, and patient progress.
•Initiates and is responsible for problem solving and decision making with relation to patient care.
•Functions as an advocate for the patient within the rehabilitation system.
•Participates in the achievement of department and hospital goals.
