Job Summary
This position is responsible for providing medically necessary behavioral health case management, care coordination, and Peer Specialist services to Medicaid enrolled consumers. Services are provided under the supervision of a mental health professional.
Job Duties/Responsibilities

- Develop individualized service plans with input from individuals seeking behavioral health services and their families and within time limits as determined by the program and best practice.
- Initiate clients into the recovery community environment, link clients to needed services, and assist individuals in the navigation of the health care system. Support clients steps towards Recovery and Wellness.
- Document case management, Peer services, and care coordination services provided to individuals and progress toward goals identified in service plans.
- Implement mechanisms that promote rapid and successful reintegration of individuals back into the community from psychiatric hospitals and long-term psychiatric placements, including participating with the hospital inpatient teams in treatment and discharge planning.
- Provide information and referrals and link individuals with community resources as needed.
- Assist enrolled consumers in obtaining state and federal assistance for which they may be entitled.
- Provide skills training, group(s) and psychoeducation to improve individuals and their families/supports understanding and management of psychiatric conditions.
- Provide behavioral health services according to state and professional ethical conduct guidelines.
- Participate in planning, provision, and evaluation of services via team meetings.
- Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and 42 CFR, Pt 2.
- Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor.
- As a mandated reporter, follow all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect .
- Perform as a team member to ensure that productivity outcome measures are achieved.
- Provide pre-crisis and post crisis support to clients as necessary. May include developing the Wellness Recovery Action Plan (WRAP).
- Carry out duties as a member of the team, under the direction of the team leader/supervisor.
- Other duties as assigned or necessary to support department.

Job Qualifications
To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job.
Education/Experience: to perform this job successfully, an individual must have an AA or bachelors degree in social services field or equivalent experience. Knowledge and/or experience in providing case management services. Or an equivalent combination of education and experience. Knowledge of community resources. Experience working with issues of mental health, homelessness, and poverty preferred.
Certificates/Licenses: Must hold a current Washington State credential (AAC, LMFT(A). LMHC(A). LASW(A), LICSW(A) and Peer Support Specialist credential. To perform this job successfully, an individual must have a valid Drivers License and the ability to drive for work use. Successfully pass background check applicable to position.
