Occupational Therapist (OT) Home Health
Location: Salem, OR (with travel required within a 60-mile radius)
Work Type: Full-Time
Compensation: PPV model, starting at $100,000/year (DOE)

Job Summary:

We are seeking a dedicated Occupational Therapist (OT) to join our Home Health team in Salem, OR. The OT will use therapeutic techniques focused on the whole patient, emphasizing personalized care to restore, rebuild, and enhance quality of life. As part of a dynamic and compassionate team, youll provide in-home therapy services to patients, improving their independence and functionality in everyday tasks.

Key Responsibilities:

• Patient Care: Assess and develop personalized treatment plans for home health patients, focusing on restoring and enhancing quality of life.

• Therapeutic Techniques: Deliver a range of therapeutic services tailored to individual patient needs in the home environment.

• Documentation: Maintain accurate and timely clinical documentation, including care plans and progress notes, in compliance with regulatory guidelines.

• Team Collaboration: Work with interdisciplinary teams to ensure comprehensive patient care and the achievement of optimal patient outcomes.

• Patient Education: Educate patients and families about therapy goals, techniques, and proper home modifications to support ongoing care.

Requirements:

• Education & Licensure:

  • Graduate of an accredited Occupational Therapy program.

  • Current state OT licensure in Oregon.

  • CPR/BLS certification.

• Experience:

  • One (1) year of recent experience in an acute care or home health setting.

  • Experience in the development of personalized care plans and patient assessments.

• Skills:

  • Excellent observation, verbal, and written communication skills.

  • Strong problem-solving abilities and basic math skills.

  • Ability to drive frequently within the designated 60-mile radius; must have a valid driver's license, auto insurance, and a reliable vehicle.

Preferred Qualifications:

• Bachelors degree in Occupational Therapy.

• One (1) year of home health care experience.

• Additional certifications or training in areas related to patient care.

Work Schedule:

• Flexible scheduling with weekends off.

• On-call rotation for one weekend per month and one day per week.

• Occasional travel required within a 60-mile radius.

Location Details:
