Occupational Therapist (OT) Home Health (Portland, OR)

Job Summary
We are seeking a dedicated Occupational Therapist (OT) to join our home health team in Portland, OR. In this role, you will provide personalized, patient-centered therapy to individuals in their homes, helping them regain skills or adapt to new challenges. As an OT, you will conduct assessments, develop individualized treatment plans, and collaborate with interdisciplinary teams to ensure optimal care and improved quality of life for our patients.

Key Responsibilities

  • Conduct thorough assessments to evaluate patients' needs and functional abilities

  • Develop and implement personalized treatment plans aimed at enhancing patients independence

  • Educate and empower patients and their families on strategies to regain lost skills

  • Collaborate with physicians and other healthcare professionals to ensure coordinated patient care

  • Maintain accurate, detailed clinical documentation and progress notes

  • Travel within a 60-mile radius of Portland for patient visits

Requirements

  • Active Occupational Therapist (OT) license in Oregon

  • Minimum of one year of acute care or home health experience

  • Valid drivers license, auto insurance, and reliable transportation

  • BLS/CPR certification

  • Excellent communication skills, both verbal and written

  • Ability to thrive in an autonomous work environment

  • Compassionate, patient-focused approach to care

Preferred Qualifications

  • Doctorate or Masters degree in Occupational Therapy

  • Previous experience in home health care (preferred)

Compensation

  • Pay per visit structure with an average annual income of $93,000 - $98,000

  • Mileage reimbursement for travel

  • Flexible scheduling: five days a week with weekends off and one weekend day per month on call

Work Environment

  • Full-time position

  • Primarily independent, in-home care

  • Requires physical activity, including standing, walking, and lifting up to 50 lbs
