Position Overview:

As a Physical Therapist, you'll provide individualized in-home therapy, helping patients maintain comfort, mobility, and independence rather than focusing on rehabilitation. You'll collaborate with an interdisciplinary care team and provide education to caregivers and families.

Key Responsibilities:

Assess & evaluate mobility, strength, and function

Develop personalized care plans with a comfort-focused approach

Provide therapeutic exercises & mobility training

Instruct caregivers & families on best practices

Document progress & communicate updates to physicians

Support hospice patients with a focus on quality of life

Qualifications:

Doctorate or master's degree in physical therapy (accredited)

Active PT license in California

2+ years of experience (hospice/home health preferred)

Strong communication & organizational skills

CPR certification required

Valid driver's license & reliable insured vehicle
