A highly respected home health provider with a multi-state presence is seeking a skilled and compassionate Physical Therapist to join its dynamic team in McMinnville, OR. As a Physical Therapist in this field-based role, you will work directly with patients in their homes, helping them regain function, mobility, and independence. Youll be supported by a multidisciplinary care team and provided with the flexibility and resources needed to deliver exceptional therapy services.

Starting Pay: $104,000$106,000 per year (paid per visit; negotiable)
Sign-On Bonus: $5,000
Coverage Area: McMinnville and Salem (travel required)

Key Responsibilities

• Provide physical therapy services under a physician-directed plan of care

• Restore or improve physical function, relieve pain, and promote patient mobility

• Instruct patients in therapeutic exercises, gait training, transfer techniques, and use of prosthetics

• Maintain accurate and timely clinical documentation

• Collaborate with interdisciplinary team members and attend care meetings

• Educate patients and caregivers on therapy goals and safety strategies

Qualifications

• Licensed to practice Physical Therapy in the state of Oregon

• Doctorate or Masters degree in Physical Therapy

• Minimum of 1 year of experience as a Physical Therapist (home health experience preferred)

• Current CPR certification

• Valid drivers license and reliable, insured vehicle
