A highly respected home health provider with a multi-state presence is seeking a skilled and compassionate Physical Therapist to join its dynamic team in McMinnville, OR. As a Physical Therapist in this field-based role, you will work directly with patients in their homes, helping them regain function, mobility, and independence. Youll be supported by a multidisciplinary care team and provided with the flexibility and resources needed to deliver exceptional therapy services.
Starting Pay: $104,000$106,000 per year (paid per visit; negotiable)
Sign-On Bonus: $5,000
Coverage Area: McMinnville and Salem (travel required)

Key Responsibilities
Provide physical therapy services under a physician-directed plan of care
Restore or improve physical function, relieve pain, and promote patient mobility
Instruct patients in therapeutic exercises, gait training, transfer techniques, and use of prosthetics
Maintain accurate and timely clinical documentation
Collaborate with interdisciplinary team members and attend care meetings
Educate patients and caregivers on therapy goals and safety strategies
Qualifications
Licensed to practice Physical Therapy in the state of Oregon
Doctorate or Masters degree in Physical Therapy
Minimum of 1 year of experience as a Physical Therapist (home health experience preferred)
Current CPR certification
Valid drivers license and reliable, insured vehicle
