Occupational Therapist (OT) Home Health

Location: West Plains, MO
Job Type: Full-Time, 13-Week Contract (with potential extension)
Schedule: MondayFriday, 8:00 AM 5:00 PM

Overview:
Sunbelt Staffing is seeking a dedicated and experienced Occupational Therapist (OT) for a 13-week contract assignment in a Home Health setting in West Plains, MO . The ideal candidate will be passionate about providing one-on-one care in the home, helping patients regain independence and improve their quality of life through individualized therapy plans.

Key Responsibilities:

• Perform initial and ongoing patient assessments

• Develop and implement personalized treatment plans

• Provide therapeutic interventions to support activities of daily living (ADLs)

• Collaborate with the interdisciplinary team to ensure comprehensive patient care

• Travel to patient homes within the local service area (mileage reimbursed)

• Maintain accurate and timely documentation in accordance with home health standards

Qualifications:

• Graduate of an accredited Occupational Therapy program

• Active OT license in the state of Missouri

• Valid drivers license and reliable transportation

• Basic Life Support (BLS) certification required

• Previous Home Health experience preferred

• OASIS experience preferred

• Familiarity with Home Care Home Base (HCHB) EMR preferred

• Strong interpersonal and time management skills
