Respiratory Therapist Care Coordinator (Pulmonary Clinic)

What you will do:

• Coordinates care across the health care continuum within designated sub-specialty population(s). Serves as single point of contact by facilitating and overseeing transitions in care within and between care settings, providers, and services.
• Perform patient visits in clinic for education, test doses, vest trials, sputum inductions, and other RT specific duties. Visit inpatients when needed for education and help with coordinating and airway clearance issues.
• Coordinates and provides patient/family education that is shared and documented regarding specific diagnosis, treatments medication management, plan of care and transitions of care.
• Completes pre-visit planning and post visit wrap-up and determines necessary follow-up appointments and/or treatments to optimize patient visit.
• Facilitates proactive disease management/preventative health care using evidenced based practice.

What you will need:

• Licensed Respiratory Therapist in the State of MN
• RRT via NBRC, or CRT via NBRC (grandfathered status with greater than 10 years of CF experience)
• Minimum 5 years of clinical experience including ICU, high acuity Respiratory Care
