As a Case Manager, you’ll play a key role in shaping the future of our services. You’ll support individuals with developmental disabilities, ensuring our high standards of care and inclusion continue to thrive. What You’ll Do:
Manage a caseload of individuals with developmental disabilities.
Lead, mentor, and support staff, including training.
Monitor daily and monthly documentation.
Facilitate regular meetings with individuals and staff.
Attend meetings with service coordinators.
Maintain open communication with Regional Center and service coordinators.
Ensure our services meet compliance standards and exceed quality expectations.
Collaborate with leadership to drive innovative, person-centered solutions.
Build strong relationships with families, stakeholders, and community partners.
Visit sites regularly to engage with staff and ensure a positive work environment.
Develop employee recognition and retention strategies, fostering a motivated, inclusive team culture.
Cover open shifts if necessary.
Communicate with, and monitor staff to ensure the quality of care.
Who You Are:
You bring valuable experience and/or education to the table. We welcome candidates who have:
A Bachelor’s Degree in Social Work, Human Services, Psychology, or a related field,
OR
At least two years of meaningful, hands-on leadership experience in human services—especially supporting individuals with developmental disabilities.
(Real-world experience is just as important to us as formal education.)
You’re passionate about making a difference in people’s lives and have a natural ability to connect with others.
You’re a confident communicator and team leader who can manage multiple priorities while supporting a positive, inclusive workplace.
You thrive in fast-paced, ever-evolving environments and adapt to change with ease.
You understand the importance of regulatory compliance and are committed to upholding high standards of care.
You’re dependable, organized, and motivated to help others succeed.
You’re willing to travel within your region as needed and have a valid driver’s license (if applicable).
Experience working directly with adults or children with developmental disabilities is a plus.
Being bilingual is also a great asset!
