Occupational Therapist

We are seeking a dedicated and experienced Occupational Therapist to join our Home Health team and help senior patients regain independence and improve their quality of life.

The Occupational Therapist (OT) provides intermittent home-based therapy services as prescribed by a physician. The OT will assess, develop, and implement treatment plans aimed at improving functional skills and promoting optimal independence.

• Administer prescribed occupational therapy treatments to senior patients in their homes.
• Conduct initial assessments and create individualized plans of care.
• Perform skilled procedures and maintain accurate clinical documentation.
• Monitor and report patient progress to physicians and adjust care plans as needed.
• Educate patients and caregivers on home programs and therapeutic exercises.
• Collaborate with interdisciplinary teams and participate in care planning.
• Supervise and guide support staff as required.
• Participate in agency in-services, QAPI activities, and promote agency mission.

• Must be a graduate of an accredited Occupational Therapy program .
• Must be a Licensed Occupational Therapist in Arizona .
• A minimum of two years of clinical experience is preferred.
• Strong interpersonal skills and documentation skills.
• Excellent communication skills and a passion for providing high-quality care.

Preferred Experience

• Previous home health care experience.
• Proficiency in upper extremity rehabilitation and activities of daily living (ADLs).
