Physical Therapist Requirements: Graduated from an accredited Physical Therapy (PT) college or university. Unencumbered Physical Therapy (PT) license for New York. Conduct home visit Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation. Proficient in EMR documentation (we use Hello Note but experience in this system is not required). CPR Certification.
